In the US, deaths due to suicide and unintentional overdose are on the rise and pose an urgent public health concern. In 2017, over 40% of suicide and overdose deaths, so-called “Deaths of Despair”, were due to opioids. This represents a 2-1/2 fold increase from 17% in 2000.1 Pain alters neurocircuitry and increases the risk of suicide, which may be unknown to many psychiatric pharmacists. There are several key shared risk factors and prevention approaches that should be known by psychiatric pharmacists. By understanding these key risk factors, pharmacists can better identify patients at greatest risk. By learning key prevention approaches, pharmacists can intervene and save the lives of patients in their care by preventing unintentional overdoses and deaths by suicide.
As a response to legislation and other pressures, many patients are having opioid therapy rapidly tapered or abruptly terminated by their providers. Many psychiatric pharmacists may be unfamiliar with various opioid tapering strategies and which is best for a given patient. Knowledge of the principles of compassionate opioid tapering can help pharmacists treat patients and educate other providers about best opioid practices.
