The California Department of Public Health (CDPH) released the Comprehensive Medication Management Programs: Description, Impacts, and Status in Southern California, 2015 white paper online. The white paper seeks to describe the current landscape, including the delivery, use, outcome, benefits, and challenges of CMM, and to describe the status of CMM implementations (“case studies”) in Southern California as of May 2015. According to CDPH, the white paper serves as a resource documenting team-based care efforts underway to make California the healthiest state in the United States of America by 2022.
