Target Audience

If you are a pharmacist, nurse practitioner, or other health care professional involved in the comprehensive medication management of individuals living with mental health and/or substance use disorders, we invite you to participate in this online course.

Session Summary

Pharmacy outside walls, or “Street Medicine,” was developed to serve individuals who are unhoused directly in their own environment to help engage this vulnerable patient population. Psychiatric pharmacists can play a pivotal role in providing direct management of the psychiatric and medical needs of patients without housing. In addition to medication management considerations, pharmacists may serve as medication experts and provide recommendations for therapy modifications for patients with sub-optimally controlled symptoms or common comorbidities. Lastly, pharmacists may assist with medication procurement for the treating team, patient access, and adherence monitoring to ensure patients have the opportunity for an optimal treatment response. This program will discuss identifying medication access barriers for this population and assess the roles of various healthcare members in street medicine teams.

Learning Objectives

 

  1. Identify barriers to medication access for individuals who are unhoused.
  2. Propose rationale for use of street medicine in the treatment of individuals who are unhoused with psychiatric illness.
  3. Assess the role of health care team members, including psychiatric pharmacists, in street medicine teams.
