These community activities focus on extremely relevant, current issues in practice and are designed to foster discussion during the calls. Please see the schedule for more information.

The Patient Care Practice Management Community is hosting a webinar on the topic of telepharmacy. Given the immense need and recent policy changes to address barriers to its use, telehealth has shifted from an underutilized service to a medical necessity almost overnight. Despite its clear utility, there still remain many unanswered questions and potential limitations of telehealth. During the webinar, speaker Caitlin McCarthy, PharmD will discuss issues related to telepharmacy and a panelist of speakers from a variety of practice setting with share their insight. Participants will learn about applications of telepharmacy and how to identify and address barriers to the provision of this vital service.

With the COVID-19 pandemic, several health care organizations have transitioned care to tele-modalities (phone or video appointments). With the transition to these previously underutilized modalities, clinicians may find themselves teleworking from home to provide patient care. Pharmacy residents may also be working remotely, and this may present a challenge for preceptors in post-graduate training programs. How to ensure adequate training and supervision of the pharmacy resident, if the preceptor is not co-located with them may be a difficult question some are struggling to answer. At the conclusion of this seminar, resident program directors and preceptors should be able to define remote or tele-precepting, recognize barriers this type of training may present and identify innovative strategies to use for remote precepting that enhance post-graduate learning.

Development of an Esketamine Administration Clinic
The presentation will briefly cover the process for the development of an esketamine administration clinic and the role of a pharmacist in the clinic. Discussion of the policies and procedures currently being utilized at the University of Maryland Medical Center will be discussed.

Acute Pain Management and Buprenorphine: Paradigm Shift
The use of buprenorphine-containing products for chronic pain or medication-assisted treatment (MAT) has greatly increased in the wake of the opioid epidemic. However, buprenorphine's unique agonist-antagonist pharmacology may cause confusion on how to treat acute anticipated or unanticipated pain. The Providers’ Clinical Support System (PCSS) for MAT attempted to bridge this gap in the literature by offering guidance on managing patients on buprenorphine-containing products during hospitalizations. The PCSS's recommendations were based largely on in-vitro and theoretical data; newer literature based on clinical practice questions previous guidelines. This clinical pearl will bring awareness to the new recommendations and provide suggestions for implementing them into your practice.

Haloperidol as a Non-opioid Alternative for Chronic Abdominal Pain/Headache in the Emergency Department
The current state of the opioid crisis led to the creation of evidence based non-opioid alternatives for acute pain management in the Emergency Department (ED). With the creation of these order sets, haloperidol was added for the treatment of chronic abdominal pain and headache. This clinical pearl will describe the mechanism by which haloperidol may be effective, summarize the data describing its effectiveness and discuss potential consequences for utilizing haloperidol for chronic abdominal pain or headache in the Emergency Department.

The PMEG Community is hosting a webinar on how to start medication groups. During the webinar we will be focusing on understanding the importance of patient medication education groups, discussing the basic steps necessary to start a PMEG, identifying resources available to aid in starting a PMEG at your facility, and brainstorming potential alternatives to in- person patient medication education groups during the COVID-19 Pandemic. The webinar is intended for new residents or pharmacists who want to learn how to start medication groups, pharmacists who already run medication groups to gain new ideas, and for everyone to join in a conversation to address challenges like COVID-19.

Calcitonin gene-related peptide (CGRP) receptor antagonists recently became an option for the treatment of acute migraines in 2020, but due to their novelty, many practicing pharmacists may not be knowledgeable of these medications. Migraines are very common, effecting as much as 15.3% of Americans 18 years and older over a 3-month period. By the end of the presentation, participants will learn how CGRP receptor antagonists work, the safety factors associated with this treatment option for acute migraines, and how to identify patients that benefit from this treatment.

The CAP Community is hosting a webinar on the topic of antipsychotic-induced metabolic effects in child and adolescent patients. During the webinar, we will be focusing on the current literature surrounding these adverse effects, including onset, duration, and any dose-dependent effects. We will also address possible treatment options when these adverse effects occur (e.g., metformin).

The Research Community is hosting a webinar on the topic of the steps in preparing a case report for publication. This webinar is appropriate for all members of CPNP. During the webinar we will be focusing on identifying a clinical relevant topic for publication as a case report. Our speaker, Geoff Brown, PharmD, is currently involved in various aspects of research and education pertaining to medical marijuana. Dr. Brown is an investigator in numerous protocols pertaining to the role of the clinical pharmacist in the management of patients using medical marijuana, and he is currently completing a case report for publication pertaining to a relevant topic in the use of medical marijuana. In addition, he has been involved in numerous studies concerning the efficacy and safety of medical marijuana. This webinar is a great opportunity for any clinical pharmacists to contribute to the knowledge base in any disease state of interest.

The Substance Use Disorder Community is hosting a webinar on the topic of pharmacotherapies for stimulant use disorder. Currently, there are no FDA-approved treatments for stimulant use disorder, though various medications have shown varying degrees of clinical efficacy. This activity will explore the evidence for several pharmacotherapies aimed at treating cocaine, amphetamine, and methamphetamine use disorder. Participants will learn about the options for treatment, how to identify patients that may benefit from pharmacotherapy treatment options, and the evidence and limitations in using medications for the treatment of stimulant use disorder.

Pre-Exposure Prophylaxis for HIV: A Primer for Psychiatric Pharmacists
Pre-exposure prophylaxis for HIV (PrEP) is highly effective at preventing new HIV infections. People with severe mental illness and people who inject drugs using shared equipment are at an increased risk of HIV acquisition. Mental health pharmacists are well-positioned to screen patients for high-risk behaviors, then appropriately educate and refer patients for PrEP. This presentation will review indications for PrEP and provide tips on how to integrate brief screening into a psychiatric medication management appointment.

Unexpected Consequences of Medication Misuse: Robotripping into Bromism
While dextromethorphan misuse carries its own health risks, it is important to note the potential for patients to develop unexpected sequalae of drug misuse such as bromism. This clinical pearl will follow a case resembling common dextromethorphan misuse with psychotic features that persisted beyond a normal course of intoxication, uncovering an underlying diagnosis of bromism. The combination of confusion or psychosis, hyperchloremia, and low or negative anion gap in absence of other causative factors should refer clinicians to consider bromism in the differential diagnosis. Clinicians should regularly assess patients with known dextromethorphan misuse for signs or symptoms of bromism.

The Cannabis Analysis: Discussing Common Contaminants in Cannabidiol-Related Products
The use of cannabidiol (CBD) products continues to increase throughout the United States despite limited data on safety and regulation of the production process. Recent product test results show CBD products are often mislabeled or inaccurately advertised to consumers. Additionally, many production processes may lead to the unintentional and intentional addition of contaminants. Given the recent rise in use and unknown efficacy and safety of CBD, this presentation will explore recent literature and product testing on CBD contamination.
