Target Audience

If you are a pharmacist, nurse practitioner, or other health care professional involved in the comprehensive medication management of individuals living with mental health and/or substance use disorders, we invite you to participate in this online course.

BCPPs desiring an evidence-based focus on an emerging area of practice and contemporary issues in psychiatric pharmacy at the specialist level are the intended audience of this recertification activity. This activity is designed for BCPPs who learn best from didactic presentations, are auditory and visual learners, and/or who learn best from hands-on-learning and dialoguing with specialist faculty and colleagues.

 

Session Summary

A critical time for patients with mental health disorders is the time of transition from hospital to community services. Patients with certain mental health disorders are at high risk for undesirable outcomes and readmission in the 30 day after hospital discharge. Pharmacists can play a critical role in transitions of care and should be involved in medication reconciliation and interventions as a member of the interdisciplinary team. Pharmacist involvement can improve medication safety, decrease readmissions rates and reduce costs. This presentation will evaluate the evidence for mental health transitions of care programs and assess the role of psychiatric pharmacists in hospital discharges.
