The Geriatric Community is hosting a clinical pearls webinar focusing on two topics: the management of delirium in the critically ill older adult and benzodiazepine withdrawal and tapering schedules in older adults.

Management of delirium in critically ill patients often represents a challenging clinical scenario. First-line strategies are largely preventative and include environmental and behavioral interventions. Guidelines recommend against the routine use of haloperidol or atypical antipsychotics, however, medications might be considered in instances of hyperactive delirium with agitation or other distressing symptoms. Given that some patients may not be able to tolerate antipsychotics or remain agitated despite their use, there is interest in the use of alternative agents such as valproic acid. This clinical pearl will provide an overview of evidence-based treatment guidelines for pharmacologic management of agitation and/or delirium in critically ill patients. Available literature for the use of off-label valproic acid for this indication will be reviewed.

Benzodiazepines have a limited role in the geriatric population considering the risks associated with long term use and the existence alternative therapies for symptom control. Managing withdrawal symptoms and taper schedules is an intimidating and complicated process that remains elusive to many pharmacists. There is conflicting evidence on the utility of switching benzodiazepine agents and the ideal rate of discontinuation tapers. Of note, outpatient resources are an essential resource in the process whether tapers are initiated in the inpatient or outpatient setting. Therefore, many tapers are unsuccessful and patients remain on these agents. There is some evidence to support the use of adjunct agents when addressing the primary diagnosis and managing the symptoms of withdrawal. The evidence on these taper techniques will be reviewed along with a few case examples.
