Course Description
Clozapine remains the most effective treatment for treatment-resistant schizophrenia. Despite well-recognized variability in clozapine metabolism and the strongest recommendation to use TDM, it still remains inconsistently incorporated into routine clinical practice. Pharmacists have long led therapeutic drug monitoring initiatives in areas such as antimicrobial stewardship, transplant medicine, and oncology, translating pharmacokinetic principles into safer, more effective patient care. With rapid clozapine testing now available, pharmacists are uniquely positioned to bring the same evidence-based approach to personalized clozapine management. The question is no longer whether clozapine levels are useful, it is how pharmacists can lead their integration into routine care.
Attendees will hear from Jordan Reese, PharmD, BCPP, who successfully led the implementation of a real-time clozapine titration and monitoring program at Colorado Mental Health Hospital in Pueblo, sharing practical lessons on overcoming barriers, building multidisciplinary support, and integrating TDM into clinical workflows.
Megan Ehret, PharmD, MS, BCPP, FAAPP, and Jordan Reese, PharmD, BCPP, will review how rapid TDM can support clinical decision-making throughout the patient journey, including dose optimization, assessment of non-response, evaluation of adherence, management of toxicity, interpretation of drug interactions, smoking-related metabolic changes, and re-titration following treatment interruption.
Participants will leave with practical tools to implement and interpret clozapine TDM programs and a deeper understanding of how pharmacists can lead practice change and advance personalized clozapine therapy.
