Mental Health Transitions of Care

Course Description

A critical time for patients with mental health disorders is the time of transition from hospital to community services. Patients with certain mental health disorders are at high risk for undesirable outcomes and readmission in the 30 day after hospital discharge. Pharmacists can play a critical role in transitions of care and should be involved in medication reconciliation and interventions as a member of the interdisciplinary team. Pharmacist involvement can improve medication safety, decrease readmissions rates and reduce costs. This presentation will evaluate the evidence for mental health transitions of care programs and assess the role of psychiatric pharmacists in hospital discharges.

Learning Objectives

  1. Evaluate the evidence for mental health transitions of care programs.
  2. Assess the role of psychiatric pharmacists in transitions of care programs.
  3. Recommend strategies to address barriers when transitioning from inpatient mental health services to outpatient primary or mental health care.

Target Audience

If you are a pharmacist, nurse practitioner, or other health care professional involved in the comprehensive medication management of individuals living with mental health and/or substance use disorders, we invite you to participate in this online course.

BCPPs desiring an evidence-based focus on an emerging area of practice and contemporary issues in psychiatric pharmacy at the specialist level are the intended audience of this recertification activity. This activity is designed for BCPPs who learn best from didactic presentations, are auditory and visual learners, and/or who learn best from hands-on-learning and dialoguing with specialist faculty and colleagues.

Dr. Bauer is a clinical pharmacy specialist at the Salt Lake City George E. Wahlen VA with clinical practice areas on the acute inpatient psychiatric unit and in mental health pharmacy transitions of care. She earned her Doctor of Pharmacy degree from the University of Tennessee Health Science Center College of Pharmacy (UTHSC-CoP) in 2013. She then worked for a year as a community pharmacist in Memphis, TN. After that, she went on to complete her PGY1 residency in Managed Care with UTHSC-CoP and the Tennessee Department of Mental Health and Substance Abuse Services (TDMHSAS) in 2015 followed by her Psychiatric Pharmacy Practice PGY2 residency at University of North Carolina Medical Center in 2016. She then worked for a year as a clinical pharmacy specialist at a local psychiatric hospital in Salt Lake City before landing her dream job in mental health transitions of care.

Dr. Bauer has served on the SLC VA’s Suicide Prevention Oversight Committee (SPOC) for several years and chaired the committee from 2020-2021. In addition, she serves on the Mental Health High Utilizer Workgroup review committee evaluating possible interventions for veterans with frequent acute care encounters.

Dr. Bauer also teaches Introduction to Neurochemistry and Pharmacology at the University of Utah College of Social Work.
