Benjamin Chavez, PharmD, BCPP, BCACP Dr. Chavez first became interested in psychiatric pharmacy during his time at the University of Florida. His first APPE rotation was at a VA hospital helping to run a clozapine clinic, and then he went on to complete a Psychopharmacology Residency at Nova Southeastern University. In 2010, Dr. Chavez shifted his focus from inpatient psychiatry to ambulatory care with a focus on behavioral health with the University of Hawaii Family Medicine Residency program. Currently, Dr. Chavez is Associate Professor at the University of Colorado Skaggs School of Pharmacy and Salud Family Health Center, a federally qualified health center, where they have implemented collaborative practice agreements for depression, anxiety, and bipolar disorder, along with protocols for chronic disease states such as diabetes, hypertension, and hyperlipidemia.. |

Austin De La Cruz, PharmD, BCPP Dr. Austin De La Cruz is a Clinical Assistant Professor in the Department of Pharmacy Practice and Translational Research at the University of Houston College of Pharmacy. Dr. De La Cruz received his Doctor of Pharmacy degree from Texas Tech University Health Sciences Center School of Pharmacy. He then went on to complete a PGY-1 Ambulatory Care Focused Pharmacy Practice Residency and a PGY-2 Psychiatric Pharmacy Residency at the North Florida/South Georgia Veterans Health System in Gainesville, Florida. Dr. De La Cruz is a Board Certified Psychiatric Pharmacy Specialist who cares for and treats individuals with mental health disorders in his outpatient psychiatric pharmacy clinic at the Michael E. William M. Ellis MS, RPh was named Executive Director of BPS in 2011. From 1997-2011, Mr. Ellis served as CEO and Executive Director of the American Pharmacists Association (APhA) Foundation. He was the Executive Vice President for the Pennsylvania Society of Health-Systems Pharmacists from 1986-1997. He received his pharmacy degree from the Philadelphia College of Pharmacy and earned his Master of Science Degree in Health Education from Saint Joseph’s University. In 2014, Mr. Ellis was appointed a commissioner to the National Commission for Certifying Agencies (NCCA) and has served as the Chairman since 2016. Kelly Gable, PharmD, BCPP Dr. Gable is a graduate of the University of Mississippi School of Pharmacy and completed her psychiatric specialty residency at the University of Southern California. Kristen Gardner, PharmD, BCPP Dr. Kristen Gardner completed her pharmacy school education at the University of Michigan College of Pharmacy. She then went on to complete her general PGY-1 residency at the University of North Carolina Hospitals and specialty training in the Western Missouri Psychiatric Pharmacy Residency Program affiliated with UMKC in Kansas City, MO. Rex Lott, PharmD, BCPP Rex practiced and taught psychiatric and neurologic pharmacy for almost 40 years. After earning a Bachelor of Pharmacy degree from Washington State University in 1972, he became an Assistant Professor in the College of Pharmacy there. He earned a Doctor of Pharmacy degree from the University of Minnesota in 1977; during that training, his interest in psychiatric and neurologic clinical practice blossomed. Since that time he has served in several different practice roles including Consultant Clinical Pharmacist for a state hospital system, Clinical Pharmacist/Pharmacy Director in two different institutions for developmentally disabled adults, long-term care Consultant Pharmacist, and private practice pharmacotherapy consultant. From 1997 until 2016 he was a full-time faculty member with Idaho State University with a practice in psychiatric pharmacy at the Boise, Idaho VA Medical Center. His practice focused on inpatient and outpatient adult psychiatry. Rex retired from teaching and active practice in 2016. Rex taught students from Idaho State University’s College of Pharmacy in both didactic course work and Advanced Pharmacy Practice Experiences in mental health. From 2008 - 2016, he taught 3rd and 4th year residents in the University of Washington’s Idaho Track Psychiatry Residency Program. He also established a psychiatric pharmacy residency at the Boise VAMC which was re-established as a PGY-2 residency in 2016. He has completed several clinical research projects. In 2016, Rex became BCPP Director for CPNP. In that position he coordinates development of CPNP’s recertification products (Psychiatric Pharmacotherapy Review Course, Clinical Application Series, Literature Analysis, From Theory to Practice Case Series, & Psychopharmacology Pearls) through 4 editorial boards. |

Andrew graduated from University of Missouri-School of Pharmacy in 2014 and completed his first year of residency at Stormont-Vail Healthcare in Topeka, Kansas. He completed his second year of psychiatric residency training at the Western Missouri Psychiatric Pharmacy Residency Program in Kansas City, Missouri. While completing his second year of residency he became interested in developing methods to improve understanding of pharmacological treatment for patients dually diagnosed with mood/affective disorders and substance use disorders. He completed a two year research fellowship through the Dallas Addiction Leadership Training Program which focused on improving treatment for patients with substance use disorders. His time in the fellowship focused on developing research methodologies that make use of resources and techniques available to pharmacists to evaluate medication effects in populations of patients historically underrepresented in published literature. His research uses epidemiologic techniques to evaluate the effects of SSRI medications on treatment outcomes in patients with PTSD and substance use disorders. To date, he has conducted the two largest investigations SSRI treatment effects in patients with PTSD and alcohol use disorder, and one of the only investigations of the effects of SSRI medications in patients with opioid use disorder. |

Holly Rabideau is a graduate of Regis University in Denver, CO, receiving a Bachelor of Science in Biochemistry (2008) and a Doctorate of Pharmacy (2014) degree. She completed a PGY1 Pharmacy Practice Residency at Stanford Health Care in Palo Alto, CA. After completion of residency, Holly remained at Stanford to help open the Stanford Neuroscience Health Center, a comprehensive ambulatory care location comprised of more than 26 subspecialties of neurology, neurosurgery and interventional neuroradiology. Her responsibilities included patient care within the infusion center, pharmacy oversight of investigational studies and patient management within the Comprehensive Epilepsy Program. Direct patient care populations included: multiple sclerosis, neuroimmunology, epilepsy, neuromuscular and headache disorders. After the birth of their daughter in 2017, Holly and her husband left California to be closer to family. In 2018, she began her current position with the System Pharmacy team at SCL Health. SCL Health is health care system with 8 hospital locations in both Colorado and Montana. In addition, Holly serves as an affiliate faculty member at Regis University where she continues to share her love of neurology while teaching multiple sclerosis and seizure disorders at the School of Pharmacy. |
