Concierge Medicine vs. Affordable Healthcare Act

In a word, more of the same. See more patients for less pay. More oversight on physicians by the multitude of stakeholders, higher taxes for those still paying taxes. Fortunately for those of us practicing or about to practice concierge medicine, only the part about paying more taxes will significantly affect us negatively. On the plus side, supply and demand theory points to an uptick in concierge medicine physician demand.

30 million new patients will need a new doctor, or nurse practitioner, or physician assistant, or pharmacist, or chiropractor, or naturopath. Unfortunately for all those patients there is already a doctor shortage. Payers, in search of profit, will continue the current downhill pay spiral. Why overpay doctors? Providers are all the same aren’t they? A widget is a widget, after all. Pay them all the same. The smart PCPs will employ many widgets to do their former jobs. Others will go out of business trying to take care of their patients. Non-concierge physicians will see more patients for less pay, more of the same. On the other hand, concierge physicians will set their own pace and their own price.
Payers want to bean-count not only quantity, but also quality, a trend that goes back as far as the eye can see. The more “quality measures,” the better. Just like a factory. Payers want to know they get what they pay for—and soon they will learn—that is exactly what they will get. Marginal providers will beef up and embellish their data to make their plastic grocery bags look like a Gucci. Then payers will be extremely happy about their documented bargains. The bag-holding patient/customer will notice that her Gucci isn’t really the Gucci she paid for. The Affordable Healthcare Act customer service rep will appropriately apologize for the inconvenience and suggest a different color Gucci. More of the same. Broccoli Lives: Healthcare Reform Avoids Constitutional Euthanasia And Survives Minor Surgery

Yesterday Americans received the US Supreme Court decision on federal health care reform. During prior oral argument there were comical references to “eating your broccoli” as a symbolic reference for doing what is healthy yet perhaps not as tasty as a cheeseburger with fries. Could the federal government make Americans “eat broccoli?” Or would the proverbial broccoli casserole get thrown out the kitchen window? The timing was momentous-a pending presidential campaign, our nation’s permanent ingrained battle over federal versus state power, and of course food: this was a fight that  Americans may have cared about more than Ann Curry’s departure from the Today show and the upcoming Olympics.

Settle down everyone, the republic survived. The US had federal single-payor health care coverage for many Americans before and after reform, and before and after this decision. The version of reform following Chief Justice Robert’s surgical removal of unconstitutional provisions expands the number of Americans who will receive federally funded healthcare.
